However, you may apply to the school for leave of absence in exceptional circumstances. Applications must be made at least two weeks in advance, in writing, using the prescribed form accessed via the link.
 

Request for school to administer medication. If your child requires medication to be administered during the school day, please complete this form. All medications must be provided in the original packaging as dispensed by a pharmacist and include the prescriber's instructions for administration. The completed form and medication must be handed into the school office by an adult. Children are not permitted to carry medication with them.
 

Pupils Leaving School Form Parents/carers are required to complete this form for any pupil leaving St Pius X (other than at the end of year 6). The information provided is essential to ensure that we know and safeguard the whereabouts of all of our pupils.
